fix(lsp): fix on_detach in lsp.diagnostic
`lsp.diagnostic.get_all()` was returning diagnotics for `:bwipeout`-ed
buffers because the diagnostic cache is not cleared. The first argument
of on_detach callback is the string "detach", not the bufnr.
