refactor(os): deduplicate console/fd boilerplate #40509
Problem:
Scattered, redundant boilerplate for Windows console/fd plumbing.
Solution:
- Add os_dup_cloexec()
- Add os_reattach_console_stdio().
- Use os_open_conin_fd().
