Before, we needed to always pack an entire msgpack_rpc Object to
a continous memory buffer before sending it out to a channel.
But this is generally wasteful. it is better to just flush
whatever is in the buffer and then continue packing to a new buffer.
This is also done for the UI event packer where there are some extra logic
to "finish" of an existing batch of nevents/ncalls. This doesn't really
stop us from flushing the buffer, just that we need to update the state
machine accordingly so the next call to prepare_call() always will
start with a new event (even though the buffer might contain overflow
data from a large event).

It is less error-prone than manually defining header guards. Pretty much
all compilers support it even if it's not part of the C standard.

Add space around arithmetic operators '+' and '-'.
Remove space between back-to-back parentheses, i.e. ')(' vs. ') ('.
Remove space between '((' or '))' of control statements.
Add space between ')' and '{' of control statements.
Remove space between function name and '(' on function declaration.
Collapse empty blocks between '{' and '}'.
Remove newline at the end of the file.
Remove newline between 'enum' and '{'.
Remove newline between '}' and ')' in a function invocation.
Remove newline between '}' and 'while' of 'do' statement.

Fixes problem introduced by “api: Allow kObjectTypeNil to be zero without
breaking compatibility”: apparently there are clients which use metadata and
there are which aren’t. For the first that commit would not be needed, for the
second that commit misses this critical piece.

- Simplify RStream/WStream API and make it more consistent with libuv.
- Move into the event loop layer(event subdirectory)
- Remove uv_helpers module.
- Simplify job/process internal modules/API.
- Unify RStream and WStream into a single structure. This is necessary because
libuv streams can be readable and writable at the same time(and because the
uv_helpers.c hack to associate multiple streams with libuv handle was removed)
- Make struct definition public, allowing more flexible/simple memory
management by users of the module.
- Adapt channel/job modules to cope with the changes.

Create the msgpack_rpc subdirectory and move all modules that deal with
msgpack-rpc to it. Also merge msgpack_rpc.c into msgpack_rpc/helpers.c
